      Hello families,

      Due to the pandemic and school closures,  20-21 registration forms are being done electronically.  For the dental form,  READ that an exam within 18 months prior to May 15 of that school year is acceptable.

      Please do not feel pressured to take your child to the dentist during the pandemic if you’re not comfortable and they have been in the required time.  Thought Id share. Surprisingly our dentist tried to have our child come in for an exam in order to complete the form.  I quickly said no as our child was just there a little over 5 months ago.  Obviously they look at us as $ only.

      advocate for your child.  And do not let your dentist make you feel they have to be seen if they were just there recently.  Our child has been going to the dentist every 6 months since 18 months old.  A delay this year should not hurt a 5 year old.

      Thanks, good information. How about the medical exam. How long is that good for?

      Physical examination requirements due upon enrollment or by Oct 15, 2020. Physical Examination must be completed within one year prior to entry to:

      1. Preschool and kindergarten (physical exam and lead screening through age 6)
      2. 6th grade and 9th grade (ages 5, 11, 15 for un-graded programs)
      3. Any student entering CPS for the first time

      *Plus the immunization requirements have to be up to date and documented

      Vision Examination requirements due upon enrollment, no later than 10/15/20

      1. Entering the State of Illinois for the first time at any grade level.
      2. Entering kindergarten

      Dental Examination requirements due 5/15/21 for kindergarten, 2nd, 6th grade and 9th grade.  Exam must be within 18 months prior to May 15, 2021 date.  So how I read this is an dental exam between Dec 15, 2019 through May 14, 2021 are acceptable by CPS.

        Great to know. thanks for sharing we did our doctor visit but will cancel our dental after chatting

          The same we did not schedule a dental exam for this month.  Kids went to dentist in Dec & Jan.  Doctor was in January 2020 with 5th birthday.  It’s a personal choice for all and there is no right or wrong answer.  I just wanted to share so families do not feel pressured to have to go to the dentist during this pandemic.  I was feeling pressured by my dentist to come in to get the form completed and be seen again.  When in fact the Dec/Jan appointment is enough. Conclusion, I advocated for the form to be completed without us coming and the dentist finally did just that.  Even if you do not have the dental done, CPS just requires it by May 2021 so if you want wait 1 year that is each family’s personal choice.  So I was never worried.
